Indigenous Services
Provided by Fraser Health
- Help individuals navigate the health care system
- Support individuals transition from hospital to home
- Work alongside health care providers to support the individuals decisions in care planning
- Connect individuals with an Elder or cultural advisor to support cultural practices
- Connect individuals with Indigenous community resources
All clients who self-identify as Indigenous or Aboriginal can receive support from an Indigenous health liaison.
