Indigenous Health Liaison - Navigation Service

Indigenous health liaisons are a key part of care teams, supporting health and wellness journeys for Indigenous clients and their families in hospital and community settings.
Indigenous health liaisons can:
  • Help individuals navigate the health care system
  • Support individuals transition from hospital to home
  • Work alongside health care providers to support the individuals decisions in care planning
  • Connect individuals with an Elder or cultural advisor to support cultural practices
  • Connect individuals with Indigenous community resources

All clients who self-identify as Indigenous or Aboriginal can receive support from an Indigenous health liaison.
