Specialized Seniors Clinics - Fraser Health Area

Provided by Fraser Health

Provides specialized services to older adults experiencing multiple medical issues and who may be at risk of losing their ability to live independently.
The Specialized Seniors Clinic does not replace an individual's family doctor or nurse practitioner and usually does not manage long-term follow-up.

Who is Served?
Support older adults who may be dealing with:
  • Chronic disease (for example, dementia, depression)
  • Risk of falls, or history of falling
  • Difficulty managing medications

  • Caregiver stress or challenges

The Approach:
The interdisciplinary team works with clients, families and their caregivers to provide:
  • Comprehensive screening and assessment

Each team member brings specialized skills. For example:
  • A pharmacist helps manage complex medication regimens.
  • A social worker supports overwhelmed caregivers.

  • Living in the Fraser Health region
  • Experiencing complex health needs that require a full geriatric assessment
