The Social Prescribing Program supports seniors aged 65 plus to access non-clinical, community-based services to prevent or delay frailty by fostering resilience and social support using a comprehensive, strength-based approach.
A local seniors' community connector with the Social Prescribing program meets with clients to help develop a personalized wellness plan to support a healthy aging journey. They connect these seniors with available community resources and liaise with your family practitioner for additional support.

The City of New Westminster began their own Social Prescribing program Summer 2024, and referral forms have not been updated yet. All referral forms for clients living in New Westminster are to be faxed to 778-312-2104.
